About the Job:
A beautifully renovated short term rehabilitation/long term care facility, seeking a Admissions Coordinator to join our dynamic team of professionals at Verrazano Nursing and Post-Acute Center . The Admissions Coordinator should have a successful track record with filling skilled nursing facility beds. In this role, you will lead the Admissions department to having a productive working relationship with local hospitals, process resident admissions in an efficient and appropriate manner, following facility policies and procedures.
Responsibilities:
Intake and Assessment: Conducting assessments of potential residents to determine their needs and eligibility for admission. This may involve reviewing medical histories, completing application forms, and coordinating with healthcare professionals.
Communication: Serving as a liaison between prospective residents, families, and the nursing home staff. This includes providing information about the facility, answering questions, and addressing concerns
Documentation: Maintaining accurate records throughout the admissions process, including required paperwork, consent forms, and medical records, ensuring compliance with regulatory requirements.
Coordination with Healthcare Providers: Collaborating with doctors, social workers, and other healthcare professionals to facilitate a smooth transition for residents into the nursing home.
Marketing and Outreach: Participating in community outreach efforts to promote the nursing home and its services, which may include attending local events or engaging with referral sources.
Orientation: Helping new residents and their families acclimate to the nursing home environment, including guiding them through the facility's policies and procedures.
Problem-Solving: Addressing any challenges that arise during the admissions process, whether related to insurance, eligibility, or specific resident needs.
Overall, the role of an Admissions Coordinator is vital for ensuring a positive experience for new residents and their families while maintaining the operational efficiency of the nursing home.
