This role provides integrated behavioral health and substance use services within primary care, supporting prevention, intervention, and recovery for diverse patients.
- Conducts evidence-based screenings, assessments, and crisis interventions, determining appropriate clinical plans.
- Creates and adjusts treatment plans, evaluating progress and ensuring access to services.
- Provides referrals, engages patients and families, and collaborates with medical staff to coordinate care.
- Documents services per policies, maintains compliance, and utilizes electronic health records.
- Maintains licensure, completes required education, and stays current with healthcare changes.
- Must be culturally competent, adaptable, and able to address disparities, with experience in outpatient mental health/substance use, preferably in primary care.
- Preferred qualifications include Maryland licensure, bilingual skills, and 2+ years of relevant experience.
