Provides integrated behavioral health and substance use services within primary care, focusing on prevention, intervention, and recovery for diverse patient populations.
- Conducts brief screenings, time-limited assessments, and emergency evaluations, applying evidence-based practices tailored to patients' needs.
- Develops and adjusts treatment plans, evaluates progress, and facilitates referrals to internal and external resources.
- Collaborates with medical providers, staff, and service teams to ensure coordinated care, maintaining clear documentation and compliance with policies.
- Uses electronic health records, adheres to licensure requirements, and participates in ongoing education to enhance skills.
- Engages culturally diverse patients, addresses disparities, and promotes diversity within interprofessional teams.
- Requires a Master’s Degree, Maryland licensure, and preferably two years of outpatient mental health/substance use experience; bilingual skills are a plus.
