Responsibilities

At our esteemed organization, we are seeking a compassionate and detail-oriented individual to join us as a Care Coordinator. In this role, you will play a vital part in assessing care plans, conducting thorough reviews of medical records, and ensuring alignment with our policies for Long Term Care (LTC) claims. Your expertise will be instrumental in determining eligibility for claimants and crafting individualized care plans that are both cost-effective and tailored to meet the unique needs of our policyholders. Additionally, you will be entrusted with the responsibility of annually signing Chronically Ill Certifications for all tax-qualified policies under claim.

As part of your duties, you will need to demonstrate exceptional research skills, meticulous attention to detail, and the ability to handle sensitive health and personal information with the utmost discretion. Effective communication is paramount in this role, as you will be tasked with conveying complex information in a clear and understandable manner to caregivers and other stakeholders.

Administrative tasks will also be part of your responsibilities, where you will handle various situations with professionalism and efficiency. Continuous training will be provided to ensure your knowledge remains up-to-date, allowing you to confidently respond to inquiries of diverse complexity.

Requirements

A minimum of 1 year of experience in care management or 2 years of clinical experience, preferably with geriatric clients, involving assessing, planning, and coordinating care.

Bachelor of Science in Nursing, along with licensure as a Registered Nurse (RN), Licensed Practical Nurse (LPN), Licensed Clinical Social Worker (LCSW), or similar licensure or degree.

Proficiency in accurately assessing policy owner needs and developing individualized care plans.

Excellent communication skills, both verbal and written, with a patient and professional demeanor.

Strong research abilities, with the capacity to synthesize information from multiple sources to support conclusions.

Understanding of the relationship between insurance claims and rates charged.

Proficiency in PC applications such as MS Word, Excel, Teams, and Outlook.

Detail-oriented mindset with a commitment to completing tasks accurately and promptly.

Ability to work independently and manage workload effectively, even under pressure.
