Case Management Specialist - #178582

Description

City Net is seeking a dedicated and empathetic Case Manager II to provide essential support to clients transitioning from street-level homelessness at the Chula Vista interim housing site. As a Case Manager, you will work towards connecting homeless individuals to sustainable housing and essential services.

Responsibilities

• Provide case management and supportive services
• Assist clients in developing long-term housing plans
• Connect clients to supportive services including medical treatment, mental health support, counseling, and permanent housing options
• Coordinate care and services with partner organizations and entities
• Operate within a Housing First and Harm Reduction service model
  
  

Requirements

• 1 year of case management experience working with populations affected by homelessness, substance use disorders, and mental health concerns
• Passion for ending homelessness and making a positive impact
• Empathetic, self-motivated, and dynamic personality
• Strong communication and networking skills
• Ability to work independently and in a team
• Proficiency in PC skills and Microsoft Office Suite
  
  

Preferred Qualifications

• Knowledge of homelessness services in Sacramento and surrounding areas
• Experience with maintaining confidential client records
• Bachelor's degree in Social Work, Human Services, Sociology, Psychology, or related field
  
  

Work Environment

• Operate from the Chula Vista office with fieldwork in various locations such as local coffee shops, restaurants, and government agencies
• Flexible working hours including evenings and weekends
• May involve driving company vehicles for client transportation

Salary Range: $23.53 - $25.53 per hour

Note: This position may require relocation to neighboring locations as needed to best serve our clients and funders.
