Athens Case Manager, Disability Services

The Case Manager is a designated Developmental Disabilities Professional who provides management for the implementation and support of the service plans for individuals with intellectual and/or developmental disabilities receiving services. The Case Manager provides comprehensive support to individuals placed in host homes and fully understands and supports the person-centered approach, community integration, and choice. The Case Manager ensures the home providers remain in compliance with Host Home requirements. They conduct at least monthly visits to monitor the health and safety of the individuals on their caseload while ensuring compliance with regulatory standards. Our Disability Services program is designed to integrate adults, adolescents and children with disabilities into community settings as active participants.

  • Located: Athens, GA
  • Remote/Virtual with some on-site required, Travel (25%)
  • Monday through Friday; 8:30AM – 5:00PM; some evening and weekend work may be required; occasional remote on-call.
  • Annual Compensation: $42,000

Responsibilities

  • Conducts site visits monthly, or more frequently as needed, and completes a management report at the time of the visit.
  • Ensures Individual Service Plans (ISP) include all information to support accurate and timely billing; confirms ISP budgets are correct for billing.
  • Ensures the Level of Care is current and signed by the Region office and participant or legal guardian; ensures Prior Authorizations are issued by the Region office.
  • Ensures functional assessments are in place to support formulation of services.
  • Monitors the Case Management System provided by IDD Connect Portal regularly to address any issues pertaining to support coordination ratings.
  • Communicate with Support Coordinators to request ISP version changes.
  • Conduct in-home or virtual Home Provider training as needed.
  • Facilitate Participant transfers as needed and ensure they are connected to the appropriate resources of their choice.
  • Maintain a working knowledge base on Comprehensive Supports Waiver Program (COMP) manual, Department of Behavioral Health and Developmental Disabilities (DBHDD) provider manuals, Department of Community Health (DCH), Policystat, and Council on Accreditation standards. Complies with federal, state and local laws that regulate our services.
  • Ensures all health and safety protocols and mitigation plans are developed and meet manual requirements.
  • Assesses the satisfaction of individuals served.
  • Ensures that current (including medication changes) are properly documented on the participant’s Medication Administration Record and coincides with all relevant in-home prescriptions (include risk/side affect training).
  • Participates in training, fundraising events, recruitment events, and other Inspiritus activities and initiatives such as quality improvement.
  • May provide management of Community Living Support (CLS) services. Conduct an initial home visit to client’s homes for service plan development. Subsequently, supervisory visits will be made every 92 days to evaluate clients and to ensure their needs are being met. Provide coaching and supervision to the Direct Support Professional, and monitor the overall satisfaction of services.
  • Fully utilize our electronic record systems to perform required compliance activities which are connected to our ability to bill for services.
  • Other duties as assigned.

Qualifications

  • BA/BS degree in Social Work, or the human services field including but not limited to: Sociology, Special Education, Rehabilitation Counseling, Psychology, etc., AND one year of experience in providing human services to individuals with Intellectual and/or Developmental Disabilities; Master’s Degree preferred, and/or established connections/experience with the I/DD community.
  • Possess a Developmental Disability Professional (DDP) designation or be eligible to obtain.
  • Excellent communication skills
  • Competent in training adult learners
  • Skilled with organizing to work independently, effectively and efficiently in a remote setting.
  • Ability to work in a professional and collaborative manner with team members, Home Providers, parents/legal guardians/family members, support coordinators, and all constituents. Ability to forge a mutually respectful partnership with persons served and their families in which they are helped to gain the skills and confidence to address any issues they face.
  • Ability to set limits and maintain the helping role of the practitioner and to intervene appropriately to mee the needs of the persons served or family members.
  • Must pass a comprehensive background check including work references, criminal background check, and TB screen.
  • Must be computer and internet literate with competencies in Microsoft Office Suite (Excel, Word, Outlook) and database/electronic case management systems.
  • Must hold and maintain a valid Georgia Driver’s license, possess a reliable personal, multi-passenger vehicle, and have personal auto insurance, and a good driving record.
