Full Job Description

The Platte County Senior Fund is seeking a compassionate and detail-oriented Case Manager to support clients through comprehensive assessments, care planning, and ongoing coordination of services. This role focuses on improving quality of life, ensuring continuity of care, and connecting clients with appropriate community resources.

Responsibilities

Client Assessment & Support

  • Conduct in-home, phone, or virtual assessments
  • Interview clients and family members to determine needs
  • Educate clients on available services and resources
  • Assess home environments for safety risks and needed modifications
  • Verify income and determine co-pay requirements
  • Maintain accurate and timely documentation

Case Management & Coordination

  • Refer clients to appropriate service providers
  • Track services, providers, and financial agreements in database
  • Follow up with clients to ensure needs are met
  • Communicate with providers regarding service quality and compliance
  • Resolve client/provider concerns and disputes
  • Maintain caseload and complete ongoing client evaluations

Administrative & Financial Duties

  • Review and process provider invoices for accuracy
  • Enter service and billing data into system
  • Assist with provider documentation (e.g., Certificates of Insurance)
  • Support bid and contract processes, including reviewing submissions

Reporting & Program Support

  • Prepare monthly activity and mileage reports
  • Assist with grant applications and reporting
  • Support audits and program compliance activities

Community Outreach

  • Build partnerships with local agencies and organizations
  • Represent the organization at community events and senior fairs
  • Provide presentations and resource information
  • Maintain and update community resources

Additional Duties

  • Supervise and support interns
  • Assist with board meeting minutes and administrative tasks
  • Provide general office support as needed
  • Perform other duties as assigned

Qualifications

  • Bachelor’s degree in Social Work (BSW), psychology, or related field, or equivalent professional experience
  • Gerontology focus or experience preferred
  • Strong organizational and communication skills
  • Ability to manage multiple clients and deadlines
  • Experience with case management or community resources is a plus
