General Duties
The primary function of the Coordinator of Community Support Services (CCSS) is to create activities for persons with disabilities who attend day program that enhance their skillset in preparation and enhancement of their community integration. Coordinator of Community Support services is responsible for the caseload of all persons attending day program including, but not limited to: Attendance at IDTs and annual meetings, upkeep of client files, communication with all stakeholders. Coordinator of Community Support Services reports to the Director of Behavioral Health
Manage CMHC Mental Health Group Treatment Program, including trauma-informed program development, program growth and maintenance, supervision of staff, and the provision of excellent consumer care using evidence-based practices for adults with mental health and co-occurring disorders.

Essential Functions
- Oversees intake services, screenings, assessment, waitlist, treatment planning, discharge planning and case management (if appropriate) of assigned clients and families.
- Assist in designing, developing and supervision of new programs and services in outpatient mental health group.
- Provide administrative supervision, consultation and clinical guidance to treatment team.
- CCSS is responsible for the timely documentation of relevant information, contacts, and action taken in the designated clinical record.
- Maintain a consistent flow of communication between Community Support Services and other Clearbrook programs as well as with client guardian.
- Plan community-based outings for Day Program clients with client input including, but not limited to: Volunteer opportunities, shopping trips, social engagement opportunities.
- Actively participate in program management and general staff meetings
- Provide clinical direction to and clinical supervision to staff, as well as LPHA approval of client’s Intergrated Assessments and Treatment Plans.
- Develop, plan, organize, direct and supervise the day-to-day activities for the CMHC day program. Communicate schedule with the CMHC department, clients, and guardians.
- Supervise the daily activities of the CSS program.
- Establish natural supports, communication skills, enhanced social skills and the completion of tasks in community setting.
- Adhere to all policies and procedures established for Clearbrook, the Community Mental Health Programs and those of all governing bodies.
- Complete mandatory training annually.
- Acts as case manager for community clients, coordinating/attendance IDTs, and intake/discharge staffings. Writing and implementing individual programs and maintaining monthly progress.
- It is the responsibility of the CCSS to maintain confidentiality with all clients and their families and maintain a professional image/role as a representative of the agency.
- Other duties as assigned by the Director of Behavioral Health.
- Ensure compliance with Clearbrook’s Personnel Policies as they relate to the programs.
- Participate as a member of an interdisciplinary team in developing an individual program plan for each assigned client.
- Master's Degree in Psychology, Counseling, or Social Work - Required
- At least 3-5 years of direct clinical experience providing individual and group psychotherapy
- At least 5 years of related experience with working knowledge of the implementation, tracking, and interpretation of clinical outcome measures.
- Experience with treating specialty populations preferred.
- Must hold a valid license in clinical psychology or social work
