Job Description

Primary Accountability

The Health Educator is responsible for collaborating with patients, family members, health-care team members, health-care team members from other organizations, and other stakeholders to reduce or eliminate barriers to care.

Description Of Primary Responsibilities

  • Builds and maintains relationships with patients and other stakeholders for the purposes of improving quality of care.
    • Such activity will include, but not be limited to, patients and their family members who have:
      • Chronic illness(es) that typically require close adherence to an ongoing treatment plan to be effective;
      • Histories of avoidable emergency department visits;
      • Prenatal needs; and
      • Other concerns or conditions that have socially mediated causes or influences that act as barriers to improving health.
    • Health Educators interface with patients and other stakeholders through a variety of mechanisms, including, but not limited to:
      • Individual, face-to-face contacts through both appointment and warm hand-off contacts;
      • Leading scheduled classes/groups within FHCN Health Centers and in the surrounding communities;
      • Telephone and other electronically mediated contacts; and
      • Contact outside of FHCN Health Centers at community functions such as health fairs.
  • During contacts with patient and family members, Health Educators reduce barriers to care in a number ways including, but not limited to:
    • Collaborating with patients and their family members in understanding, setting and monitoring self-management plans;
    • Assisting patients with finding resources, services and support mechanisms for their care plans and self-care management needs;
    • Provides the patient with educational materials and resources appropriate to the health literacy needs, including assisting patients in understanding how to successfully access care both within and outside of FHCN; and
    • Assisting patients with assessments, orientations, educational plans, referral processes, and forms completion relevant to programs that serve special populations including but not limited to:
      • Comprehensive Perinatal Services Program, including high risk perinatal patients;
      • Title X and Family PACT services; and
      • Cancer Detection/Every Woman Counts program.
  • During contacts with health-care team members at FHCN and from other organizations, Health Educators reduce barriers to care in a number of ways including but not limited to:
    • Preparing, printing and distributing the information necessary for care teams to engaging in Morning Huddles;
    • Maintaining and servicing panels of patients with special service needs, as determined by health center or organizational need;
    • Maintaining regular communication with care team providers on patient care plan goals and progress;
    • Facilitating regular communication between patient and other health-care team members both inside and outside FHCN; and
    • Providing staff training and education sessions necessary to implement health education services.
  • Participates in meetings and trainings as necessary to facilitate the above duties, including those geared toward implementing evaluation tools that determine the effectiveness of Health Educator functions.
  • Completes necessary reporting and documentation associated with the above activities per organizational and regulatory requirement.
  • Performs other duties as assigned.
Description Of Primary Attributes

Professional & Technical Knowledge:

  • Possesses work-related skills at a higher level than completion of high school, including formal written and verbal communications skills, computational and computer skills, mathematical, technical or health care related knowledge frequently acquired through completion of a trade school, para-professional, certificate type program, or equivalent work experience.
  • Bachelor’s Degree in related field preferred.

Technical Skills

  • Ability to prepare more complex documents in Microsoft Word, including creating tables, charts, graphs and other elements.
  • Ability to use Microsoft Excel to review and compile data, including the use of formulas, functions, lookup tables and other standard spreadsheet elements.
  • Ability to create basic presentations in Microsoft PowerPoint.

Licenses & Certifications: None required.

Other Requirement: As a condition of employment at Family HealthCare Network, all employees assigned to work with the Cutler Orosi School District must pass a Federal Background Check.

Communications Skills

  • Job duties require the compilation of information prepared in effective written form, including correspondence, reports, articles or other documentation.
  • Effectively conveys technical information to non-technical audiences.

Pay Scale

Min Hourly Rate: $21.25

Max Hourly Rate: $29.33
