ROLE OVERVIEW And PURPOSE
Health Information Coordinator will assist and coordinate administrative and operational activities for PACE medical records. This role will work with PACE Clinic teams, PACE management, Compliance Officers, and IDT members on activities related to medical records and keeping track of care plans, participant enrollment, and dis-enrollments processes. Additionally, this role will maintain control of participant clinical and non-clinical records.

Pay range: $23.85-28.50/hr per hour, depending on qualifications.

ROLE OVERVIEW And PURPOSE
Health Information Coordinator will assist and coordinate administrative and operational activities for PACE medical records. This role will work with PACE Clinic teams, PACE management, Compliance Officers, and IDT members on activities related to medical records and keeping track of care plans, participant enrollment, and dis-enrollments processes. Additionally, this role will maintain control of participant clinical and non-clinical records.
Responsibilities
- Tracks medical records and serves as subject matter expert for electronic medical record (EMR) systems
- Responds and coordinates with departments regarding requests for system support, such as IT or outside vendors for assistance in resolving system issues
- Processes and scans reports from charts into EMRs in permanent file area
- Release records according to confidentiality guidelines
- Removes inactive charts from permanent file area and relocates to offsite locations
- Answers phones, operates fax machines, and assists providers with medical records, as needed
- Impact participant experience by demonstrating courteous and helpful behavior and a commitment to accuracy
- Works closely with management and the Compliance Department to create, update, and implement medical records
- Facilitates and keeps minutes of meetings, as needed
- Assists MSWs with dis-enrollments and participant documentation
- Prepares and tracks service request documents and composes denial of service letters, as needed
- Maintains logs for tracking participant service request appeals and denials
- Maintains awareness and reports potential fraud, waste, and abuse to management, compliance officers, or Neighborhood’s compliance hotline
- Liaises between Neighborhood and external medical records vendors and contractors
- Tracks and coordinates participant institutionalizations, discharges, etc., including updating information as needed in the EMR
- Communicates changes in participant condition to IDT
- Communicates and interacts with patient caregivers and/or family members, as needed
- High school/GED required
- Medical compliance, ethics, patient confidentiality, and administration training preferred
- One year experience working with elderly populations required
- Six months medical records experience required
- Bilingual (English/Spanish) preferred
- Valid BLS certification in accordance with the American Heart Association Guidelines required upon hire
- Excellent verbal and written communication skills, including superior composition, typing and proofreading skills
- Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
- Knowledgeable about and experience with providing care for geriatric patients
- Ability to apply analytical and critical thinking skills to resolve patient information issues
- Ability to successfully manage multiple tasks simultaneously
- Excellent planning and organizational ability
- Ability to work as part of a team as well as independently
- Ability to work with highly confidential information in a professional and ethical manner
- Ability to lift/carry 10 lbs/weight
- Ability to stand for long periods of time
Pay range: $23.85-28.50/hr per hour, depending on qualifications.
