Under supervision of the Hospice Team Manager and AVP Operations, act as assistant to the Hospice interdisciplinary group members and coordinate the provision of ancillary services.
Job Responsibility
- Order laboratory, x-ray, medical supplies and equipment for patients and document.
- Accurately enters all ancillary services information into the EMR.
- Fax lab results to patient's primary care physician and relay information to Team Manager and Hospice Care RN for follow-up.
- Contact patient /family to verify equipment and/or supply delivery.
- Process new patient admissions; existing patient discharges/transfers and deaths.
- Prepare agenda for Interdisciplinary Team (IDT) Meetings and attend meetings to scribe.
- Assist Team Manager and IDT members with various miscellaneous assignments.
- Prepares patient and family for discharge and follow-up care.

- High School Diploma required, and equivalent combination of education and related experience.
- 1-3 years of relevant experience, required.
