They facilitate referrals, coordinate with medical staff, and serve as a key liaison within the healthcare team.
Key responsibilities include building and maintaining relationships with physicians, social workers, case managers, and discharge planners.
Qualifications include a current NYS Registered Nurse license, with a preference for a Bachelor's Degree.
Minimum of two years hospice nursing experience, proficiency in physical assessment and interviewing, and excellent communication skills are required.
Advanced critical thinking, problem-solving, independence, and bilingual Spanish/English abilities are also essential, depending on location.
