Position: MDS Coordinator
Location: Delray Beach, FL
Shift: Full-Time, Days
Salary: $38 - $45 per hour
Key Responsibilities:
- Coordinate and complete MDS assessments accurately and within required timelines.
- Develop, implement, and update individualized care plans tailored to resident needs.
- Ensure documentation compliance to support PDPM and Medicare billing requirements.
- Collaborate closely with interdisciplinary teams to ensure high-quality resident care.
- Analyze Quality Indicator/Quality Measure (QI/QM) data and assist in performance improvement initiatives.
- Support preparation for surveys and audits, ensuring readiness and compliance.
- Ensure timely electronic submission of all MDS assessments.
- Provide training and ongoing support regarding MDS updates and best practices.
- Active Registered Nurse (RN) license in the state of Florida.
- Strong knowledge of PDPM and Medicare billing processes is essential.
- Minimum of 2 years’ experience in a skilled nursing facility preferred.
- Excellent communication, problem-solving, and teamwork skills.
