The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers. To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.
Program Overview
The Bureau of Equitable Health Systems (BEHS) is a bureau in the Division for Center for Health Equity and Community Wellness at the NYC Department of Health and Mental Hygiene (DOHMH) located at 42-09 28th Street, Long Island City, NY. Our division is devoted to strengthening the Department’s ability to strategically partner with the NYC health care system. The bureau will engage primary care providers and other healthcare organizations to implement evidence-based strategies; leverage information to support planning and technical assistance for providers and payers; advance policy to close the racial equity gap for priority health outcomes; and surface opportunities where health care can influence and connect consumers to social support and address the whole person, beyond physical ailments. The Community Healthcare Support Team (CHST) within BEHS operates NYC REACH, helping New York City providers to implement new health care delivery models and quality initiatives, participate in payment reform programs, and connect to community and health resources to support their patients. Position Overview
BEHS is seeking a Program Coordinator to support program staff and partners in the implementation of best practices to assist quality improvement initiatives which maximize prevention opportunities in primary care. The Program Coordinator will support aspects of the Cardiovascular Health Innovation Program (CHIP), a CDC grant-funded program aimed to identify and respond to health disparities in cardiovascular disease and improve related outcomes in high-risk neighborhoods across New York City. This position will work under the guidance of the Programs and Administration Manager and Community Healthcare Support Team leadership to assist in tracking team and grant deliverables to ensure program activities are completed as scheduled. The Program Coordinator will develop and maintain CHST communications by organizing trainings and meetings, creating agenda, disseminating materials, and ensuring follow-through on action items. This role will coordinate across NYC REACH and BEHS in various projects and duties to ensure alignment with project goals, CHS team goals, and the health department’s overall mission and goals.

Responsibilities
- Coordinate program and project efforts in the development and implementation of quality improvement strategies
- Assist with project management and grant-related activities
- Attend off/on-site meetings, as necessary
- Assess and document program activities and team progression
- Provide administrative support; schedule and develop meeting agendas, meeting minutes, material preparation
- Develop presentations for internal and external stakeholders
- Other duties as assigned
- Bachelor’s Degree
- 2+ years of relevant experience
- Ability to work collaboratively with physicians, staff and external organizations
- Excellent time management, attention to detail, and organizational skills required
- Excellent verbal, written, presentation, remote communication and group facilitation skills
- Working knowledge of Microsoft Word, Excel, PowerPoint and Outlook
- Knowledge about chronic disease prevention/ management is a plus
- Annual Salary range is $ 60,000 to $ 65,000
