Key Responsibilities
- Assess, plan, implement, and evaluate individualized resident care plans
- Administer medications, treatments, and nursing interventions as prescribed
- Monitor resident conditions and promptly communicate significant changes to physicians and appropriate team members
- Maintain accurate, timely, and compliant clinical documentation
- Coordinate care with physicians, therapists, nursing staff, and interdisciplinary team members
- Provide clinical guidance and supervision to CNAs and other nursing personnel
- Ensure compliance with all federal, state, and facility regulations
- Promote resident dignity, comfort, and quality of life through compassionate care
- Respond effectively to emergencies and changing resident needs
- Participate in care planning, quality improvement initiatives, and resident advocacy
- Current and active California Registered Nurse (RN) license
- Current CPR/BLS certification
- Strong clinical assessment, critical-thinking, and decision-making skills
- Excellent communication and interpersonal abilities
- Ability to work effectively in a fast-paced healthcare environment
- Experience in skilled nursing, post-acute care, rehabilitation, or long-term care preferred
- Passion for delivering exceptional resident-centered care
