Position Overview
We are seeking an experienced, self-driven Full-Time Registered Nurse (RN) to join our dedicated Home Health team and provide exceptional care throughout the region. As an Registered Nurse (RN), you will evaluate patients' health, create and implement care plans, and track their progress to maintain the highest standards of care. You'll be working closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field.

- Provide nursing care within the scope of practice, following the patient’s individualized care plan, in coordination with the Interdisciplinary Team.
- Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
- Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
- Supervise Licensed Vocational Nurses (LVNs) when assigned.
- Comply with all company policies and procedures to ensure high standards of care.
- Current Registered Nurse (RN) License to practice in California - Required
- Experience in Home Health or Hospice care - 1 Year Required
- Current CA Driver’s License
- Able to travel within a 15 - 25 mile radius on a regular basis.
