The Social Services Designee/Activity Coordinator will work directly under the supervision of the Director of Social Services to assist in planning, developing, organizing, implementing, and evaluating our facility's Social Services and Activities programs.
Social Services Designee/Activity Coordinator Responsibilities
Social Services
- Participate in developing facility social work policies.
- Participate in quality assurance interdisciplinary team meetings.
- Understand and meet all government requirements for social service documentation.
- Document progress in meeting the psychosocial needs of residents.
- Work with the interdisciplinary team and administration to promote and protect resident rights and the psychological wellbeing of each resident.
- Prevent and address resident abuse as mandated by law and professional licensure.
- Work with residents, families, significant others and staff to provide support, information and organization for taking a more proactive role in self-advocacy to improve quality of life/care for individual residents and those who live and work within the nursing home and the community at large.
- Participate in the development of a written, interdisciplinary plan of care for each resident that identifies the psychosocial needs/issues of the resident, the goals to be accomplished for those needs/issues, and the appropriate social worker interventions.
- Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to a long-term care facility, including their social, emotional, and psychological needs.
- Ensure or provide support and education to residents’/family members/significant others to assist in their understanding or placement and facility issues in addition to referring them to appropriate social service agencies when the facility does not provide the needed services.
- Provide groups for residents’/family members/significant others as appropriate to their needs.
- Coordinate the resident discharge planning process and make referrals for appropriate home care services prior to the resident’s return to the community.
- Educate residents and families/significant others regarding their rights and responsibilities, effective problem solving and the extent of community, health and social services that are available to them, including those necessary for effective discharge planning.
- Compile information on discharge plans and present to appropriate committee as required.
- Compile and maintain a social service directory listing available community resources.
- Coordinate monthly Behavioral Meeting with nursing Unit Manager and psych related stakeholders to monitor and track resident behaviors, and necessary GDRs
- Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
- Meet with administration, medical and nursing staff, and other related departments in planning social services, as directed.
- Maintenance and collaboration with Social Services Director related to Grievance Log reporting, and record keeping with timely follow up.
- Make routine visits to residents and perform services as necessary.
- Partake in State Reportable investigations as assigned including initial reporting, witness statements/staff interviews, resident interviews, and writing of summary.
- Routine check-in/check-out with Social Services Director, at the discretion of the Social Services Director.
- Participate in completing appropriate social services sections of the MDS/CAAs as necessary.
- Works beyond normal working hours, on weekends, and in other positions temporarily, when necessary.
- Maintains a liaison with other department supervisors to adequately plan for social services/activities while being an active leader/organizer of Fun Committee.
- Assist and enroll residents in ancillary services (podiatry, audiology, dentist, hygienist, optometrist, psych services, etc.) and maintaining schedules, consult notes, and follow ups as ordered.
- Other duties as assigned.
- Daily visitation on a one-to-one basis for residents on the One-to-One program.
- Selection of appropriate programs for the residents on the one-to-one program such as crafts, table games, service projects, intellectual activities, etc.
- Lead small, medium, and large group activities, as well as a balance of programs held on the varied floors and in the central activity room.
- Offer a balance of recreational, intellectual, religious service and community orientated programs.
- Execute daily group exercise on floors where feasible, individual exercises when group sessions are not possible.
- Lead evening and weekend programs.
- Document daily recording of resident’s participation in activities.
- Charting residents progress quarterly or more often as needed including activities related MDS/CAAs.
- Attendance at in-service classes and workshops, both within and outside the facility and department conferences with consultant at the discretion of the Social Services Director.
- Contacting each new resident and familiarizing them with the activity opportunities available to them.
- Assist in the formation of the monthly activity schedule and in the evaluation of all programs.
- Responsible for the daily preparation of materials and supplies needed for the day’s programs and for informing the Administrator of supplies that need to be acquired for upcoming programs.
- Coordination of goals, schedule, and program as not to clash with, but rather assist with all other departments.
- Lead and keep records of meeting minutes for Resident Council on a monthly or as needed basis.
- Assist residents with voting and elections.
- Attendance of care conferences when invited.
- Outreach to family’s and coordination of Family Council.
- Monthly activity calendar; managing creation of and posting of the monthly activity calendar for the facility and within each resident room.
