Key Responsibilities
Care Coordination
- Serve as the main liaison among clients, caregivers, nurses, and family members.
- Conduct intake calls and perform initial assessments to initiate care services in alignment with client needs.
- Develop and update individualized care plans in collaboration with clinical staff.
- Track client progress and adjust care plans proactively.
- Manage care transitions, such as discharges from hospitals or skilled nursing facilities.
- Respond promptly to client concerns and ensure resolution to maintain client satisfaction.
- Maintain accurate and up-to-date documentation in compliance with California state regulations and HIPAA.
- Build and maintain caregiver schedules to ensure full coverage across all cases.
- Match caregivers with clients based on care needs, preferences, and availability.
- Respond to last-minute call-outs or changes with speed and professionalism.
- Ensure compliance with labor laws, licensure, and staffing requirements.
- Oversee daily operations of the office, including supply inventory, file management, and system maintenance.
- Supervise administrative support staff and foster a collaborative, high-performing office culture.
- Support billing, payroll, and timekeeping processes in coordination with the finance team.
- Assist with recruitment, orientation, and ongoing training for office and field staff.
- Monitor documentation for compliance and support internal audits and reporting.
Education & Experience
- Associate or Bachelor’s degree in Nursing, Healthcare Administration, Business, or a related field preferred.
- Minimum 2 years of experience in Home Care, Home Health, Hospice, or a healthcare coordination role.
- At least 1 year in a supervisory or office management position within a healthcare setting preferred.
- Strong working knowledge of California home care and home health regulations (Title 22, HCO, CMS, etc.).
- Skilled in care plan development, compliance documentation, and scheduling systems.
- Exceptional interpersonal and communication skills—both verbal and written.
- Proficient with Microsoft Office Suite, EMRs (e.g., Axxess, WellSky, or similar), and scheduling platforms.
- Able to prioritize and manage multiple responsibilities in a dynamic environment.
- Calm under pressure and able to mediate conflicts with empathy and professionalism.
- Being bilingual in English/Spanish or another language is a strong plus.
- Based in our San Ramon office with occasional visits to client homes or facilities as needed.
- May require participation in on-call rotation or availability for after-hours issues.
