Provides strategic and operational oversight of post-acute care services across contracted and affiliated providers, with a primary focus on long-term care and nursing facilities participating in MPAP and QIPP programs. Leads the monitoring and analysis of clinical and non-clinical performance data to ensure compliance with regulatory standards and to drive quality improvement initiatives. Develops and maintains collaborative partnerships with post-acute providers to strengthen care transitions, enhance continuity across the care continuum, and support cost-effective care delivery. Serves as a subject matter expert to internal and external stakeholders, aligning post-acute strategies with organizational goals related to quality, compliance, and system performance.

Essential Functions

1. Oversees QIPP program performance across affiliated nursing facilities, ensuring alignment with state-defined quality metrics and program requirements.

2. Monitors and analyzes clinical and operational data (e.g., MDS, QAPI, readmissions, infection control) to track performance against QIPP measures and identify improvement opportunities.

3. Leads development and execution of Quality Assurance and Performance Improvement (QAPI) initiatives to meet or exceed QIPP quality benchmarks.

4. Ensures compliance with federal and state regulations, including CMS requirements and Texas Medicaid guidelines, impacting QIPP participation and performance.

5. Coordinates preparation for regulatory surveys and manages corrective action plans related to deficiencies that may impact QIPP scoring or funding.

6. Collaborates with nursing facility administrators to implement standardized workflows and best practices that improve quality outcomes and QIPP incentive attainment.

7. Provides education and ongoing training to facility leadership and staff on QIPP measures, documentation standards, and regulatory expectations.

8. Oversees accuracy and integrity of data submissions (e.g., MDS and supporting documentation) that directly influence QIPP performance and reimbursement.

9. Develops and maintains policies, reporting structures, and internal controls to support sustained compliance and optimal performance within the QIPP framework.

10. Partners with internal stakeholders to align post-acute strategy with system-level goals related to quality outcomes, readmission reduction, and value-based care performance under QIPP.

Education Required: Must have a Bachelor's degree in Nursing

Preferred: Advanced degree in nursing, healthcare administration or other related fields

Work Experience Required:

3+ years
  • Hospital or community-based patient care nursing
  • Long Term Care experience

Preferred:
  • Two years working with the most recent version of the Resident Assessment Instrument and Minimum Data Set, Quality Assurance Performance Improvement initiatives, Medicare and Medicaid Claims processing.
  • Nursing home surveying experience
  • Multi-site home health and hospice experience
  • Healthcare administration experience
  • Broad regulatory knowledge of home health, nursing home, skilled nursing facility and hospice agency regulation and administration.

Licenses/Certification(s)/Registration(s) Required:
  • Must have current, valid RN license or temporary RN license from the Texas Board of Nursing, or valid Compact RN license.
  • Must have current healthcare provider BLS for Healthcare Providers certification from one of the following:
    • American Heart Association
    • American Red Cross
    • Military Training Network

Preferred
  • Surveyor Minimal Qualification Test Certification, (SMQT), Quality Indicator Surveyor (QIS) Certification, Accreditation for Care Management (ACM) from American Care Manager Association, American Nurses Credentialing Center (ANCC) Nursing Case Management Certification, or National Academy of Certified Care Managers (NACCM)
